Before you begin, please note:

1. This form MUST be filled out by a custodial parent or legal guardian.

2. A separate application must be completed for each child you want to register

3. Use the button at the bottom of the form  to submit your information and proceed to the next step of the application. Your application is not considered by our team until all sections have been submitted.

4. You'll be asked to complete a $800 non-refundable deposit. This deposit will tentatively hold your space on the program, subject to submission of your initial forms. 

5. This application takes around 20 minutes to complete.
